I tried, but my camera isn't that sharp that close up! Here's a transcription though.
Front cover:
THE REPORT BOOK
Containing the Reports of
Samantha Parkington
Miss Crampton's Academy
For the School Year 1904-1905
Inside front cover:
TO PARENTS.
This Report is sent to Parents for their inspection
directly after the close of each school month. After
having inspected the items reported, please write
your name in the proper space on the last page of
the book, and return to [sic] the teacher. Should there
be any delay in the presentation of this report, or
any neglect in its preparation, you will confer a favor
by calling attention to it immediately.
As this is intended to be a complete summary of
all that pertains to your child's school life this year,
it is hoped that you will regard it with more than a
mere passing interest, and that you will co-operate
with the teacher in securing the best possible re-
sults. The value of the Report Book will largely de-
pend upon the interest shown in it by you. The pupil
naturally looks for praise or censure, according as
the record is good or bad. If good, an encouraging
word should be spoken. On the other hand, if irreg-
ular attendance, low deportment, or neglected les-
sons be reported, the fact should not pass unnoticed.
Should you at any time discover anything of an
unsatisfactory character, through means of these
reports or otherwise, it is earnestly desired that you
will call upon and consult with the teacher at once.
By pursuing this course, misunderstandings will be
avoided, and the parties most interested in the wel-
fare of the pupil will mutually assist each other.
Miss S. Stevens

TO THE TEACHER.
This little book is intended to contain all the
Reports necessary to be made regarding the At-
tendance, Deportment and Progress of the pupil
herein named. See to it that it is well kept. Fill
the blanks regularly and carefully at the close of
each month. Send them to parents promtly at
the time indicated on the second page of the
cover, and see that they are returned with the
proper signature.
Make all entries with pen and ink, and in such
a way that no one can fail to understand. Exercise
such care that erasures shall never be necessary.
The manner in which these Reports are made
will be a good index to the general character of
your school.
This book is arranged for either Primary or
High Schools. When used in the High School, the
leaf containing the names of the Primary and
Grammar School branches may be removed by
cutting off at the dotted line. The report of Ex-
aminations in the High School branches, and At-
ttendance and Summary, will then face each other.
If rightly used, this book will prove to be a
valuable medium of communication between you
and the parents. It will stimulate the pupil to do
his best work at all times, and it will otherwise
assist you greatly in securing desired results at
school.

Lol, I started out thinking, "this'll be so easy!" But once I had started I wanted to finish it, since I'm sure many others have had the same problems reading it.
I know, it's so interesting to see what the girls' grades are. Sam's drawing of her gingerbread train station in Samantha's Surprise - that looks pretty good to me! Lol. But then again I can't draw. In Samantha Saves the Day, it does mention how she and the twins have a hard time making their paintings turn out the way they want them to.

In the printed report card it looks as if extra subjects were added by the school- what were they.French is a possibility.
Samantha probably got marked down in art because she used her imagination. She did not make an exact copy of what that were to draw.
Thanks for your work

Yes, the last three examinations subjects were written in by "Miss Stevens." They are French, Sewing, and Physical ed.
Lol, yes, Samantha is the imaginitive type, I guess! Maybe she's just an artist ahead of her time.
